Is Ibogaine Safe? An Honest Look at the Risks, Deaths, and Safety Protocols

February 10, 2026 · Safety

Let's start with the most important thing: ibogaine can be dangerous. It has caused deaths. Any website that doesn't tell you this upfront is not a source you should trust.

There are approximately 33 ibogaine-related deaths documented in the scientific literature. That number deserves context — but it also deserves respect. Each of those deaths represents a real person, and understanding why they happened is essential to making an informed decision.

The primary risk is cardiac. Ibogaine causes QT interval prolongation — a change in the heart's electrical activity that can, in rare cases, trigger a fatal arrhythmia. In plain language: ibogaine temporarily affects your heartbeat, and if you have an underlying heart condition, this can be dangerous.

The vast majority of documented fatalities occurred in unregulated settings: no medical staff present, no cardiac monitoring, no pre-treatment screening, impure ibogaine products, or concurrent use of other substances (particularly methadone and other drugs that also prolong the QT interval).

In properly screened patients treated at medically supervised facilities with continuous cardiac monitoring, the risk profile is dramatically different. The Stanford MISTIC study, for example, used a magnesium-ibogaine protocol with comprehensive screening and monitoring — and reported no serious cardiac events.

This doesn't mean the risk is zero. No medical treatment is without risk. But it means that the difference between a safe ibogaine experience and a dangerous one largely comes down to the quality of the clinical setting.

Other common side effects include nausea (very common — most patients experience this), headache, coordination difficulty, tremors, and visual disturbances during the treatment itself. These typically resolve within 24-72 hours. Less commonly, some individuals may experience psychological disturbances, particularly those with predispositions to psychotic disorders.

The bottom line: ibogaine safety is not about the substance alone — it's about the context in which it's administered. A well-run clinic with proper screening, monitoring, and medical staff can manage the risks effectively. An unregulated setting cannot.
